Office Policies

Office Hours

 

Our office hours are 8:00 AM to 5:00 PM Monday through Thursday.  In the event of an emergency, we can be contacted 24 hours a day.

 
Appointments

 

Every effort will be made to be on time for your child's appointment.  Children are frequently subject to accidents.  In order to accommodate an injured child, our schedule may be delayed.  Please accept our apologies ahead of time should such a delay occur during your child's appointment.  Understand we will do exactly the same if your child is ever in need of emergency care. 

 

Late afternoon appointments are reserved for middle and high school children.  Children in preschool and primary grades are usually seen in the morning, as they are well rested and are able to have a more pleasant experience.  School absence forms will be provided if needed.  No charge will be made for rescheduling your appointment provided 48 hours notice is given to us so that your child's time can be given to another patient.  There is a $50.00 fee charged for a broken appointment if we are not notified 24 hours in advance.  For new patient appointments if there is a late  cancellation or broken appointment, we will not reappoint.

 

Fees

 

All professional fees are due at the time of service.  Payment can be made by cash, check, VISA, MasterCard, Discover or Care Credit. There is a $20.00 fee for all returned checks.

 

Please be aware that the parent bringing the child to our office is legally responsible for payment of all charges.  We cannot send statements to other people.

It is Dr. Dail's policy to inform all patients of the fees for the treatment planned. If for any reason we fail to inform you of the fees, it is your responsibility to ask.

 

Insurance

 

As a convenience to you, our staff will assist in processing your insurance forms.  Please bring your insurance information to the first appointment.  New patients are asked to pay for the first visit at the time of appointment, whether or not they are covered by insurance.  For our patients with dental insurance, we will accept payment for further treatment directly from your insurance carrier, however, we ask you to pay the estimated non-covered fees as treatment progresses.  Remember that you, and not your insurance carrier are ultimately responsible for payment of services rendered in our office.

  • Co-payments and deductibles are due at the time of service.

  • Any unanticipated amounts, not paid by your insurance company, are your responsibility.

  • After (60) days, all outstanding balances, including outstanding insurance claims, are due and payable by you.

  • As a courtesy to you, we will handle the insurance billing; however, it is not our obligation.

  • If your insurance company requests information from you, you must respond in a timely manner.

  • We recommend that you keep track of your benefits (such as maximums or eligibility requirements).

  • It is your responsibility to make sure your insurance is paying what they are supposed to. We will assist you as needed.

  • All recommended treatment is determined to be necessary for the oral health of your child. Failure of your insurance company to pay does not indicate the treatment can wait nor waive any of our fees.
